The command could not connect to the database `DATABASE_URL` names.

## Cause

The command could not connect to the database `DATABASE_URL` names.

The connection failed before the command did anything. The Why line is the database driver's own reason: a refused connection, an unknown host, a failed login or a missing database.

## Fix

* Check that the database is running, and reachable from where the command runs.
* Check the host, port, database name, user and password in `DATABASE_URL`.
* Then run the command again.
