Cause

The database refused a statement of a migration or of a schema change.

The database refused a statement of a migration, or of a schema change. Inside a transaction nothing of the migration stays applied; the Why line gives the database's own reason.

Fix

• Fix the statement, or the data it fails on, as the Why line says.
• Run the command again; --verbose shows the statement and where it failed.
